Panniculectomy vs Tummy Tuck — Which Is Right for You?
Patients with a hanging lower abdominal skin fold (panniculus) often ask which procedure is most appropriate. The distinction is clinically important.
A removes the panniculus — the overhanging skin and fat fold below the umbilicus — without altering the abdominal muscles or repositioning the umbilicus. Its primary indication is functional: the panniculus causes persistent skin irritation, moisture trapping, rashes, fungal infections and mobility difficulties. Removing it allows patients to move normally and exercise without constant skin problems. It does not tighten the abdominal wall or produce a flat stomach contour.
A addresses the full abdomen — removing excess skin and fat, tightening the abdominal muscles where they have separated, and repositioning the umbilicus to produce a flatter, more contoured abdominal profile. It is a cosmetic and functional procedure.
Where the concern is both functional (skin overhang causing irritation) and cosmetic (wanting a flatter, tighter abdomen), abdominoplasty is the more appropriate procedure. Panniculectomy alone is most appropriate where the patient’s primary concern is relief of the functional symptoms caused by the overhanging skin.

Excess skin removal procedures are appropriate for patients with loose, sagging skin that does not respond to diet and exercise. General suitability criteria:
For patients who have lost more than 25 kg, high-protein dietary supplementation for at least six weeks before and after surgery is recommended to reduce the risk of wound healing complications.

Preparation for Excess Skin Removal Surgery
Once a consultation has taken place and a surgical plan agreed, the preoperative assessment team will provide written preparation instructions specific to your procedure. Key requirements:
Stop smoking at least four weeks before surgery and for four weeks after. The risk of wound breakdown and infection is significantly higher in smokers — this is particularly important for procedures involving long incisions and skin flap elevation such as tummy tuck and body lift surgery.
Stop aspirin and aspirin-containing medications at least one week before surgery. Stop anti-inflammatory medications such as ibuprofen unless directed otherwise. Avoid herbal supplements for at least one week.
Do not eat or drink for six hours before your procedure. Clear fluids — still water, black tea or black coffee — are permitted up to two hours before.
Arrange a responsible adult to take you home and remain with you for the first 24 hours after surgery. For larger procedures (tummy tuck, body lift), prepare a comfortable recovery area at home where you can rest with everything accessible without bending or stretching.

Your consultation takes place at our Baker Street clinic with a GMC Specialist Register consultant plastic surgeon and takes approximately 45–60 minutes. Your surgeon will take a full medical history, review all medications and conduct a physical assessment of all areas affected by loose skin. They will evaluate the degree and distribution of skin excess in each area, the skin quality, the presence of underlying muscle laxity, and any residual fat deposits. Based on this assessment they will advise on the most appropriate procedure or combination of procedures. Where multiple areas require treatment, your surgeon will discuss the recommended staging plan — the sequence in which procedures should be performed and the appropriate interval between sessions. You are not required to commit to all stages at the first consultation; each stage is consented separately with its own cooling-off period. Your surgeon will explain the incision positions, expected scar locations, procedure durations, recovery protocols, and all risks and potential complications. High-resolution photographs are taken. A mandatory two-week cooling-off period applies from the date you provide consent before surgery can be booked.
